Do not have insurance

Income Guidelines
The current guidelines state that you must have a household income at, or less than, 250% of the federal poverty level.
Currently the limits are:
1 person = $39,900 per year
2 persons = $54,100 per year
3 persons = $68,300 per year
4 persons = $82,500 per year
What to Bring
Patients must make an appointment - we cannot see walk-ins.
Proof of residence and income is required.
New patients must present a verifiable form of identification at their first appointment.
