    The primary mission of Volunteers in Medicine’s practice is to enhance human well-being and help meet basic human needs with special attention to people and communities who are vulnerable, oppressed, and/or living in poverty.

    VIM patients, their families, and other clients in the community are eligible to receive support through these programs. There’s no cost to schedule an appointment with a social worker.

    VIM’s Social Services team offers the following services:

    • Social Work

    • Patient Navigation

    • Patient Assistance Program (PAP)

    Through these programs, social workers can assist with the following:

    • Psychosocial assessments

    • Social Determinants of Health (SDoH) assessments

    • Support through the Community Information Exchange (CIE)

    • Internal and external (community) referrals

    • Collaboration and support with other interdisciplinary teams

    Our team can also help with referrals to:

    • State-Provided Legal Resources

    • Access to Federal (SSA) and State/County benefits

    • Temporary disability assistance and State Disability Insurance (SDI)

    • Employment Development Department (EDD) assistance

    • Food, housing, and emergency assistance shelter resources

    • In-home support services

    • Caregiver support and resources

    • Transportation coordination and resources

    • California Lifeline assistance (energy bills and phone bills)

    • Housing/Emergency Shelter Resources

    • And other resources based on the needs of the patient and their family.

    To set-up an appointment with a social worker, ask your provider during your next visit or call (619) 440-4591. Our trained Patient Navigators can assist you in scheduling an appointment. Appointments are available in-person or over the phone.
